For your convenience, we accept Visa, MasterCard, Discover, personal checks or cash. We do not participate in any medical HMO and PPO plans. We do not participate in Medicare.

If you do not have dental insurance, or if you are unable to provide the insurance information: then full payment is required at the time of service.

If you do have dental insurance: 1.) If charges are $300 or less, full payment is due at the time of service. 2.) If charges are greater than $300, a 40% deposit is due at the time of service.

Regardless of insurance status: Any balances over 90 days will be turned over for collection.

Regarding delinquencies: There will be a $30 service charge on all returned checks. Any delinquent account after 3 months will turned over for collection and you will be responsible for all legal and collection fees.

Failed Apppointments: Without 24-hour notification, involving general anesthesia, may incur a $100 service fee.

We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered.  If you have questions regarding our fees or the status of your account, please contact us at (708) 460-9100. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

If you have insurance, we will be happy to file your claim for you. When the insurance claim is processed and payment is received, we will credit your account accordingly.  If you have underpaid and still owe a balance then we will send you a bill.  If you have overpaid and have a credit balance then we will promptly refund the balance to you.
